Complete the assigned intake form accurately and review preparation, cancellation, rescheduling, refund, and package terms before submitting payment.

Pregnant clients are not eligible for body or Jet Plasma treatments. Other answers may require individual review or an adjusted treatment plan.

“Not sure” is a valid starting point. That is exactly what the optional consultation is for.
